Abstract. Vulvar cancer in the structure of the incidence of malignant neoplasms of the female genital organs is about 2-5%. The five-year survival rate for the early stage is about 80-90%. Despite aggressive treatment, the risk of recurrence remains high and reaches 43%. The determining factor of progression is the presence of metastases in the sentinel lymph nodes and further adequate management of these patients. The article reveals the main results of the largest GROINSS-V study, which showed the clinical safety of the sentinel lymph node assessment procedure, as well as the justification for replacing inguinal-femoral lymphadenectomy with adjuvant radiation therapy in patients with sentinel lymph node metastases less than 2 mm.
