Abstract. Bouveret syndrome is an extremely rare form of gallstone small bowel obstruction. The disease is associated with high mortality – from 12 to 30%, since the main cohort of patients is the elderly age group, and the non-specificity of clinical symptoms and laboratory tests leads to late diagnosis. In addition, due to the fact that the disease is very rare, there are currently no officially approved recommendations for diagnosis and priority treatment. The main role in the effectiveness of treatment is given to a timely and interdisciplinary approach to the choice of treatment, since open surgical interventions are associated with high mortality rates. The article presents a clinical case of successful endoscopic treatment of mechanical intestinal obstruction due to Bouveret syndrome.

Key words: Bouveret syndrome, small bowel obstruction, biliary fistula, gallstone, endoscopic treatment.
