Abstract. Sarcomas are extremely rare tumors of the pancreas accounting for less than 0,1% of all pancreatic malignancies. Pancreatic sarcomas tend to be more aggressive and have a poor prognosis. As for primary undifferentiated pleomorphic pancreatic sarcoma, there are only 16 such cases in the medical literature. A 17th case of primary undifferentiated pleomorphic sarcoma of the pancreatic head in a 61 year old man with symptoms of high small bowel obstruction and mechanical jaundice, which ended fatally 22 days after palliative surgery, is presented. The issues of clinical and histopathological diagnostics are discussed, a brief review of the literature is given.
