LIVER TRANSPLANTATION IN TATARSTAN

Abstract

Aim ― to analyze the short-term and first long-term results of orthotopic liver transplantation in the Republic of Tatarstan.

Material and methods. From December 2018 to December 2021, 58 cadaveric orthotopic liver transplantations were performed. Among the recipients there were 37 women and 21 men, the average age of the patients was 46,55±10,9 years (from 21 to 66 years).

Results. Over the course of three years, 58 orthotopic liver transplantations were performed under the conditions of Republican Clinical Hospital (1 operation in December 2018, 12 operations in 2019, 18 operations in 2020, 27 operations in 2021). The main cause of liver cirrhosis in recipients was autoimmune diseases with liver damage. The average MELD score was 15,24±4,5 (from 7 to 29). Surgical complications in the postoperative period occurred in 15 patients (25,9%). In the postoperative period, 6 patients died (10,3%, 30 and 90-day hospital mortality). The average length of stay was 27,5±14,2 bed-days, the average length of stay in the intensive care unit was 8,8±8,6 bed-days. Taking into account the timing of the liver transplantation program in Tatarstan, the one-year survival rate among patients was 87,5%. Thus, in 2021, the provision of the region was 6,9 orthotopic liver transplants per 1 million population (with a target of 20).

Conclusion. The results of the implementation in the Republic of Tatarstan of the liver transplant program in the treatment of terminal stages of its chronic diseases make it possible to look with optimism to the future of organ transplantation in the region. The priority tasks are to increase the number of operations performed, to create an interregional liver transplant center.
