Abstract. The article assessed the effect of stress associated with the volume of surgery on the quality of life in 274 patients with stages I-IIIA breast cancer aged 18 to 40 years. Before the operation, a decrease in the quality of life indicators characterizing the emotional state of women with breast cancer was revealed. Six months after the operation, the indicators of the quality of life of patients with breast cancer who underwent mastectomy with simultaneous breast plastic surgery did not differ significantly from those of healthy patients with breast plastic surgery. In the group of patients with radical mastectomy, the quality of life indicators were significantly lower: their RP decreased to 43,1±4,8 points against 79,3±4,8 points in the group of healthy women with breast plastic surgery and 72,1±5,3 points in the group of patients with breast cancer who underwent radical mastectomy with simultaneous plastic surgery. The level of cortisol in the blood and vanilyl mandelic acid in the urine before surgery was increased in all patients. After 1 year and after 5 years, their level decreased. The work proved that in breast cancer the volume of surgical intervention has a significant impact on the psychological state of women and their quality of life. A radical mastectomy dramatically reduces quality of life. Women who have undergone radical mastectomy are in a state of chronic stress. Immediate breast plastic surgery improves the quality of life.

Key words: breast cancer, quality of life, simultaneous breast reconstruction, cortisol.
