Abstract. The research is based on the results of treatment of 323 patients with IV stage (T3-4N0-2M1a-1b) rectal cancer. In the case of synchronous resection of distant metastases (M1a) and a primary tumor, 5-years survival rate: in the main group (R0) was 36.89%, in the first comparison group (R1) ― 10,28%, in the second comparison group ( R2) ― 2,63%. In the case of synchronous resection of two or more distant metastases (M1b) and a primary tumor: overall 5-years survival rate was 15.6% (R0), in the first comparison group R1 ― 7.4%, in the second comparison group with cytoreduction R2 ― 0%.
